Are Buenos Aires and Montevideo walkable from the ship?
Montevideo is: the port sits beside the old town. In Buenos Aires the cruise terminal is inside a working port area, so you will need a shuttle, taxi or tour to reach the neighborhoods.
Is São Paulo a realistic day trip from Santos?
Yes, but it is a long day. The drive up the coastal mountains takes well over an hour each way, so a guided tour or private driver is the most practical way to see the city and return on time.
